The Blues were behind early on when Jordan Obita headed home a penalty and then scored his own goal after being fouled by Gary Cahill.
But they fought back through two goals from Willian, with the first coming from a penalty awarded for a foul on Pedro in the box and the second coming from a deflected shot from Cesc Fabregas.
Fulham thought they had won it when substitute Josh Maja scored from close range but Willian's header secured all three points for Chelsea.
It was a tough afternoon for Fulham, who have now lost four of their last five games and are currently bottom of the table.
Manager Slavisa Jokanovic said: "We did not play well enough today. We started very slowly and we were always under pressure.
"We have to work hard to improve our performance."
Meanwhile, Willian was delighted to score twice for Chelsea, saying: "I'm happy that I could help the team win the game. It was a difficult situation against a good team like Fulham.
"It's important to keep fighting even if you're down. The players showed great character."
